Michigan Central Registry Clearance Request

 

 

The State of Michigan now requires Central Registry Clearance checks to be renewed annually. Pioneer Scout Reservation will only be accepting clearance forms that are dated on or after January 1, of the current year of camp. If you have a clearance that is dated before the current year of camp you must renew this form before attending summer camp!

A new form has also been provided to allow users to email their forms in and receive their clearance back via email. THIS IS THE PREFERRED METHOD AS IT WILL TAKE SIGNIFICANTLY LESS TIME TO RECEIVE YOUR CLEARANCE BACK. Mailed and faxed forms are also still accepted.

 

Pioneer Scout Reservation is located on the Ohio/Michigan state line with many portions of the camp located within the State of Michigan. The State of Michigan requires all adults (21 and older) attending summer camp to go through a Central Registry Clearance with the Michigan Department of Licensing and Regulatory Affairs, Bureau of Community and Health Systems ON AN ANNUAL BASIS.

Instructions:

  • Adults 21 years of age and older attending camp, download the form below and fill it out completely.
  • Send in the form via email, mail, or fax. IT IS HIGHLY RECOMMENDED TO SUBMIT AND REQUEST YOUR CLEARANCE BACK VIA EMAIL TO RECEIVE IT BACK IN THE QUICKEST AMOUNT OF TIME. If requesting the clearance to be mailed back, USE YOUR HOME ADDRESS AND NOT THE CAMP ADDRESS! ONLY PROVIDE CAMP WITH A COPY OF THE CLEARANCE!
  • Turn in a copy of each clearance with your other paperwork at the pre-camp leaders meeting or when arriving at camp.
