Summer Camp Required Forms

The forms below are required to attend summer camp at Camp Frontier.

Required For All Youth

BSA Health and Medical Record (Parts A, B, and C)

Informed Consent, Release Agreement, and Authorization

Required for All Adults Attending Less Than 72 Hours

BSA Health and Medical Record (Parts A and B)

Informed Consent, Release Agreement, and Authorization

Michigan Central Registry Clearance

- Must also Include proof of current Youth Protection Training (i.e. printed certificate or copy of pocket card)

Required for All Adults Attending 72 Hours or More

BSA Health and Medical Record (Parts A, B, and C)

Informed Consent, Release Agreement, and Authorization

Michigan Central Registry Clearance

- Must also include proof of current BSA Membership (i.e. printed unit charter or copy of current BSA membership pocket card)
